10/6/2023 EnchantaVaughn Approved (AM)Upon arrival, client entered the classroom in a hostile and disruptive manner. Client did not start his work right away. Client was confronted and sent to have one on one with his counselor. Client stated he didn't get much sleep the night before and was not in a good mood. Client was encouraged to lose the attitude and get his work done before he's penalized for his actions. Client promised to be more respectful and comply to classroom rules so that he can earn a break. Client failed to complete his assigned task as he was observed talking to a few of his buddies in class. Client was off task as he discussed non- related school topics with his friends in class. Client was asked to reframe from talking and get back to his work. Client became frustrated and immediately caught an attitude. QMHP went to assist and get him back on track. Client was receptive to reinforcement and resumed his class activity. Teacher praised client for all of his efforts but was very disappointed at him today. Teacher did not understand what went wrong and why client was acting so ignorant. Client was inquired about his feelings. At first he put all the blame on his teacher, then he finally broke his silence and opened up to QMHP. Client verbalized he did not want to be in the class this morning and felt he was being targeted. Client was reminded that it isn’t fair to mistreat others because he’s not enjoying himself. Client was educated that it hinders the whole class when the teacher cannot teach. Client was also educated that he is not being targeted but is expected to try his best since he does receive additional help at school. Client was ready to go back to class and make better choices. During the final period client appeared to have a transition in his demeanor as he was able to complete his assigned task and dismiss in a more content manner.
